Sacrospinous ligament fixation treats prolapse of the upper vagina or womb. The vault or cervix is stitched to the sacrospinous ligament, a strong band of tissue in the pelvis. It is done entirely through the vagina, without mesh.

How the surgery works
Under anaesthetic, a cut is made in the vaginal wall to reach the ligament. Strong stitches attach the vault or cervix to it. It is often done alongside a vaginal hysterectomy or a front or back wall repair. The operation usually takes one to two hours.
Who may benefit
Women with prolapse of the upper vagina or womb who prefer a vaginal approach without mesh, or who are less suited to longer abdominal surgery. It is also used during a vaginal hysterectomy to help prevent a later vault prolapse.
What to expect
Most women stay one to two nights. Some buttock pain on one side is common for a few weeks and usually settles. Recovery takes around six weeks, and you will have a review visit.
