Treating severe endometriosis when keyhole surgery is not enough

Most endometriosis is treated by laparoscopy. Open surgery, through a cut in the lower tummy, may be needed for very severe disease, dense scarring, large endometriomas or disease involving the bowel or bladder. It may also be combined with a hysterectomy.

How the surgery works

Under general anaesthetic, the pelvis is opened through a lower tummy cut. Endometriosis and scar tissue are removed, organs are freed and cysts are treated. If the bowel or bladder is involved, another surgeon may join the operation. Hormone treatment may follow to lower the chance of the disease returning.

Who may benefit

Women with severe or deep endometriosis, previous unsuccessful surgery, or disease affecting other organs, where a keyhole approach is not safe or suitable. It may also be considered when symptoms continue despite earlier keyhole surgery.

What to expect

You will usually stay in hospital for three to five nights. Recovery takes around six to eight weeks. You will have regular reviews to monitor your symptoms and adjust treatment.
