An abnormal semen analysis is a starting point, not a diagnosis. A full assessment looks for reasons such as a hormone imbalance, a varicocele, infection, a blockage or a genetic cause. Some of these can be treated directly.

Receiving an abnormal result can be upsetting. Knowing the cause often opens up more options.

What the assessment covers

A detailed history covers illness, medicines, surgery and lifestyle. An examination checks the testes and looks for a varicocele. Blood tests measure hormones such as testosterone and FSH. Genetic tests, a scrotal ultrasound or tests for infection may follow, depending on the results.

Who may benefit

Men with a low sperm count, poor movement, abnormal shape or no sperm in the semen. It also helps men with a history of testicular problems, hormone treatment or anabolic steroid use.

What to expect

Your first visit is a private conversation and examination. Tests are arranged over the next few weeks. At your review, you will receive a clear explanation of the cause, where one is found, and the options for treatment.

What is a male infertility assessment?

It is a detailed check to find the cause of an abnormal semen result, including hormones, genetics and physical factors.

Who should have one?

Men with an abnormal semen analysis, or with a history that may affect fertility, such as testicular surgery or steroid use.

What tests are involved?

Hormone blood tests, a physical examination and sometimes genetic tests, a scrotal ultrasound or infection screening.

Is it uncomfortable or risky?

Most tests are blood tests and scans. The examination is brief and private. There are no significant risks.
