Azoospermia Assessment & Treatment
When no sperm are found in the semen, careful tests show whether sperm are being made, and whether they may be retrieved for treatment.
Male Fertility
Finding the cause when no sperm are seen
Azoospermia means no sperm are found in the semen. There are two main types. In the first, sperm are made but a blockage stops them getting out. In the second, the testes make very few sperm or none.
This result can come as a shock. Many men with azoospermia still have options, including surgical sperm retrieval.
Dr Jasdev explains the tests step by step, and what each result means for you as a couple.
Both partners are assessed
Male factors play a part in many couples’ fertility problems, so sperm health is checked early.
Results explained clearly
You will understand what each result means and which options follow from it.
A plan for you as a couple
Male and female findings are considered together, so decisions make sense for both of you.
What a full assessment gives you
- A clear answer on the type of azoospermia
- Checks for hormone, genetic or blockage causes
- A plan for sperm retrieval and ICSI when suitable
How your care is planned
Each stage answers what is happening, why it matters, and what comes next for you as a couple. The plan is reviewed as results come in.
Confirm the result with a repeat semen analysis
Arrange hormone, genetic and ultrasound tests
Discuss medical treatment or surgical sperm retrieval
How azoospermia is assessed
A repeat semen test, with the sample spun down, confirms the finding. Hormone tests, the size of the testes and a scrotal ultrasound help show whether sperm are being made. Genetic tests, such as a Y chromosome check, may be advised. Treatment depends on the type found.
Who may benefit
Men with no sperm on a semen analysis, including after cancer treatment or a vasectomy. It also applies to men with very low counts where sperm are seen only occasionally. It also helps men who had cancer treatment in childhood and now want to know their options.
What to expect
Tests are arranged over a few weeks. At your review, you will learn the likely cause and the options. If surgical sperm retrieval is suitable, it is often planned to match your partner’s IVF cycle, or sperm is frozen in advance.

What causes azoospermia?
Is surgery needed?
Could our child inherit the cause?
Your questions about azoospermia
What is azoospermia?
It means no sperm are found in the semen. It can be caused by a blockage or by low sperm production in the testes.
Who should be assessed?
Any man with no sperm on a semen test. A repeat test is always done to confirm the result.
What tests are needed?
A repeat semen analysis, hormone blood tests, a scrotal ultrasound and sometimes genetic tests.
Is treatment painful or risky?
The tests are simple. If sperm retrieval surgery is needed, it is done under anaesthetic, with some soreness afterwards. Risks are explained before any procedure.
